O excesso de gordura corporal está, freqüentemente, associado a alterações metabólicas importantes e às doenças crônicas, como diabetes, hipertensão arterial e dislipidemias. A disponibilidade de métodos que permitam estimar a composição corporal é fundamental para avaliação de atletas e do estado de saúde de indivíduos. Há diversos métodos para análise da composição corporal, e é importante conhecer a limitação de cada um e a sua validade para que as estimativas obtidas sejam confiáveis e interpretadas corretamente. Este estudo tem como objetivo revisar a aplicabilidade de alguns indicadores do estado nutricional e de métodos de avaliação da composição corporal, tais como índice de massa corporal, circunferência da cintura e relação cintura-quadril, medidas de dobras cutâneas, bioimpedância elétrica e interactância de infravermelho próximo.]]></p></abstract>

An excess of body fat is frequently associated with important metabolic alterations and chronic diseases, such as diabetes, hypertension and dyslipidemia The availability of methods to assess body composition is essential to evaluate health status of individuals.There are several methods for the corporal composition analysis, and it is important to know the limitation of each one and his/her validity so that the obtained estimates are reliable and correctly interpreted. The purpose of this study was to revise the applicability of some nutritional state indicators and corporal composition evaluation methods, such as corporal mass index, waist circumference and waist-hip relation, skinfold measurements, bioelectrical impedance analysis and near-infrared interactance.]]></p></abstract>

O excesso de gordura corporal e</font> <font COLOR="#231f20" size="2" face="Verdana">sua distribuição centralizada se destacam pela influência no</font> <font COLOR="#231f20" size="2" face="Verdana">aparecimento das doenças crônicas não-transmissíveis,</font> <font COLOR="#231f20" size="2" face="Verdana">principalmente as doenças cardiovasculares (1).</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Há uma série de métodos para a avaliação da composição</font> <font COLOR="#231f20" size="2" face="Verdana">corporal, que variam segundo suas bases físicas, custo,</font> <font COLOR="#231f20" size="2" face="Verdana">acurácia, facilidade de utilização e de transporte do</font> <font COLOR="#231f20" size="2" face="Verdana">equipamento. Os métodos mais sofisticados e considerados</font> <font COLOR="#231f20" size="2" face="Verdana">mais precisos, como a pesagem hidrostática e a absorção do</font> <font COLOR="#231f20" size="2" face="Verdana">raio X de dupla energia (DEXA), permitem quantificar oscomponentes corporais, e a tomografia computadorizada e a</font> <font COLOR="#231f20" size="2" face="Verdana">ressonância magnética quantificam a gordura localizada. Todos</font> <font COLOR="#231f20" size="2" face="Verdana">eles possuem uso limitado na avaliação de grupos</font> <font COLOR="#231f20" size="2" face="Verdana">populacionais, devido ao elevado custo e à complexidade, e</font> <font COLOR="#231f20" size="2" face="Verdana">têm utilização restrita a laboratórios e em situações clínicas</font> <font COLOR="#231f20" size="2" face="Verdana">muito específicas (2).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Em estudos populacionais, indicadores antropométricos,</font> <font COLOR="#231f20" size="2" face="Verdana">como índice de massa corporal (IMC), circunferência da</font> <font COLOR="#231f20" size="2" face="Verdana">cintura e relação cintura-quadril, e métodos de avaliação da</font> <font COLOR="#231f20" size="2" face="Verdana">composição corporal menos complexos, como medidas de</font> <font COLOR="#231f20" size="2" face="Verdana">dobras cutâneas e bioimpedância elétrica, são amplamente</font> <font COLOR="#231f20" size="2" face="Verdana">empregados devido à praticidade e ao baixo custo, apesar de</font> <font COLOR="#231f20" size="2" face="Verdana">não fornecerem uma avaliação tão acurada e detalhada, já que</font> <font COLOR="#231f20" size="2" face="Verdana">são utilizados, na maioria das vezes, sem prévia validação na</font> <font COLOR="#231f20" size="2" face="Verdana">população que se pretende avaliar.</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Considerando-se que o excesso de gordura corporal está</font> <font COLOR="#231f20" size="2" face="Verdana">relacionado ao aumento da morbi-mortalidade, principalmente</font> <font COLOR="#231f20" size="2" face="Verdana">em adultos, para a avaliação são necessários métodos precisos</font> <font COLOR="#231f20" size="2" face="Verdana">e confiáveis, de baixo custo e grande aplicabilidade em estudos</font> <font COLOR="#231f20" size="2" face="Verdana">populacionais e na prática clínica.</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Este estudo teve como objetivo revisar a aplicabilidade de</font> <font COLOR="#231f20" size="2" face="Verdana">alguns indicadores do estado nutricional e de métodos de</font> <font COLOR="#231f20" size="2" face="Verdana">avaliação da composição corporal, tais como, índice de massa</font> <font COLOR="#231f20" size="2" face="Verdana">corporal, circunferência da cintura e relação cintura-quadril,</font> <font COLOR="#231f20" size="2" face="Verdana">medidas de dobras cutâneas, bioimpedância elétrica e</font> <font COLOR="#231f20" size="2" face="Verdana">interactância de infravermelho próximo.</font></p> <b>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Revisão de literatura</font></b> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">A associação entre excesso de peso corporal e aumento da</font> <font COLOR="#231f20" size="2" face="Verdana">morbi-mortalidade tem sido demonstrada em vários estudos</font> <font COLOR="#231f20" size="2" face="Verdana">epidemiológicos (3,4). Dados da World Health Organization(5) mostram que a doença isquêmica do coração e o acidente</font> <font COLOR="#231f20" size="2" face="Verdana">vascular cerebral estão em primeiro lugar entre as causas de</font> <font COLOR="#231f20" size="2" face="Verdana">mortalidade. Diante desta estimativa e do fato de a obesidade</font> <font COLOR="#231f20" size="2" face="Verdana">ser fator de risco importante para doenças crônicas nãotransmissíveis,</font> <font COLOR="#231f20" size="2" face="Verdana">o seu diagnóstico é de fundamental importância</font> <font COLOR="#231f20" size="2" face="Verdana">para o direcionamento das ações de saúde e redução da morbimortalidade.</font></p>     ]]></body>
<body><![CDATA[<p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" face="Verdana" size="2">Análises dos dados do National Health and Nutrition</font> <font COLOR="#231f20" face="Verdana" size="2">Examination Survey (NHANES) confirmam mudanças na</font> <font COLOR="#231f20" face="Verdana" size="2">distribuição do IMC na população americana com um aumento</font> <font COLOR="#231f20" size="2" face="Verdana">médio de 4,6 pontos, entre 1890 e 2000, em homens com idade</font> <font color="#231f20" size="2" face="Verdana"> entre 40 e 49 anos, e de 6,3 pontos, neste mesmo período, emhomens na faixa etária de 60 a 69 anos, resultando em aumento</font> <font COLOR="#231f20" size="2" face="Verdana">da prevalência de obesidade de 3,4% para 35% (6). No Brasil,</font> <font COLOR="#231f20" size="2" face="Verdana">observa-se também a crescente prevalência de obesidade em</font> <font COLOR="#231f20" size="2" face="Verdana">todas as regiões do País e principalmente nos estratos de renda</font> <font COLOR="#231f20" size="2" face="Verdana">mais baixa (7).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Normalmente, os métodos e pontos de corte utilizados no</font> <font COLOR="#231f20" size="2" face="Verdana">diagnóstico da obesidade são propostos por comitês</font> <font COLOR="#231f20" size="2" face="Verdana">internacionais, como a World Health Organization, entretanto</font> <font COLOR="#231f20" size="2" face="Verdana">a prevalência de obesidade varia consideravelmente segundo</font> <font COLOR="#231f20" size="2" face="Verdana">os parâmetros de cada comitê. </font> <font COLOR="#231f20" size="2" face="Verdana">Esta variação ocorre porque,</font> <font COLOR="#231f20" size="2" face="Verdana">na maioria das vezes, a população que está sendo avaliada</font> <font COLOR="#231f20" size="2" face="Verdana">apresenta características bastante diferentes da amostra em que</font> <font COLOR="#231f20" size="2" face="Verdana">o método e/ou ponto de corte foram validados. Além davariabilidade biológica, a adoção de protocolos e de</font> <font COLOR="#231f20" size="2" face="Verdana">metodologias diferentes normalmente contribui para as</font> <font COLOR="#231f20" size="2" face="Verdana">diferenças nas estimativas relatadas na literatura.</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Estudos para identificar métodos mais adequados e pontos</font> <font COLOR="#231f20" size="2" face="Verdana">de corte capazes de predizer o risco de alterações metabólicas</font> <font COLOR="#231f20" size="2" face="Verdana">têm sido desenvolvidos em todo o mundo (8-10). Entretanto,</font> <font COLOR="#231f20" size="2" face="Verdana">em nosso País, estudos de representatividade nacional, que</font> <font COLOR="#231f20" size="2" face="Verdana">envolvem grupos específicos da população, são escassos</font> <font COLOR="#231f20" size="2" face="Verdana">(11,12). São usados métodos e pontos de corte validados em</font> <font COLOR="#231f20" size="2" face="Verdana">outras populações, com aplicabilidade muito limitada devido</font> <font COLOR="#231f20" size="2" face="Verdana">a uma série de diferenças relacionadas à etnia, ao nível de</font> <font COLOR="#231f20" size="2" face="Verdana">atividade física e à idade.</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">A adoção de pontos de corte arbitrários, aliada à utilização</font> <font COLOR="#231f20" size="2" face="Verdana">de métodos que não foram validados para a população</font> <font COLOR="#231f20" size="2" face="Verdana">brasileira, faz com que sejam estabelecidas estimativas errôneas</font> <font COLOR="#231f20" size="2" face="Verdana">da composição corporal, resultando tanto em diagnósticos</font> <font COLOR="#231f20" size="2" face="Verdana">quanto em medidas de intervenção equivocadas em relação</font> <font COLOR="#231f20" size="2" face="Verdana">ao estado nutricional de indivíduos e populações.</font></p> <b>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Antropometria</font></b> <b><font COLOR="#231f20" size="2" face="Verdana">IMC, Circunferência da cintura e relação cintura-quadril</font></p> </b>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Até 1980, os padrões de peso ideal eram baseados emtabelas de peso e altura específicas para cada sexo, elaboradas</font> <font COLOR="#231f20" size="2" face="Verdana">pela Metropolitan Life Insurance Company. Posteriormente,</font><font COLOR="#231f20" size="2" face="Verdana">o IMC passou a ser utilizado e, desde então, vem sofrendo</font> <font COLOR="#231f20" size="2" face="Verdana">modificações no que diz respeito aos pontos de corte e número</font> <font COLOR="#231f20" size="2" face="Verdana">de categorias para diagnósticos por diferentes organizações</font> <font COLOR="#231f20" size="2" face="Verdana">como World Health Organization, National Center of Health</font> <font COLOR="#231f20" face="Verdana" size="2">Statistics/Centers for Disease Control and Preventions e</font> <font COLOR="#231f20" face="Verdana" size="2">National Heart, Lung, and Blood Institute (13).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">O IMC é atualmente um dos indicadores antropométricos</font> <font COLOR="#231f20" size="2" face="Verdana">mais utilizados na avaliação do estado nutricional de</font> <font COLOR="#231f20" size="2" face="Verdana">populações e em estudos epidemiológicos, com a finalidade</font> <font COLOR="#231f20" size="2" face="Verdana">de explorar a associação entre obesidade e várias doenças,</font> <font COLOR="#231f20" size="2" face="Verdana">principalmente as cardiovasculares (14, 15). Entretanto,</font> <font COLOR="#231f20" size="2" face="Verdana">existem muitas limitações com relação ao seu uso, já que ele</font> <font COLOR="#231f20" size="2" face="Verdana">não é capaz de fornecer informações sobre a composição</font> <font COLOR="#231f20" size="2" face="Verdana">corporal e a distribuição da gordura corporal (16). No estudode Yao et al. (17), por exemplo, mais de 30% dos indivíduosque apresentaram excesso de gordura corporal foramclassificados como eutróficos pelo IMC, demonstrando sua</font> <font COLOR="#231f20" size="2" face="Verdana">baixa sensibilidade na identificação do excesso de gordura</font> <font COLOR="#231f20" size="2" face="Verdana">corporal.</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">A baixa sensibilidade do IMC em detectar indivíduos com</font> <font COLOR="#231f20" size="2" face="Verdana">excesso de gordura corporal também foi constatada em um</font> <font COLOR="#231f20" size="2" face="Verdana">estudo em que os indivíduos foram avaliados por</font> <font COLOR="#231f20" size="2" face="Verdana">bioimpedância. Em torno de 61% das mulheres e 23,6% doshomens com IMC &lt; 30kg/m² apresentaram percentual de</font> <font COLOR="#231f20" size="2" face="Verdana">gordura corporal maior que 30% e maior que 25%,</font> <font COLOR="#231f20" size="2" face="Verdana">respectivamente (18).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">A utilização desse método se torna bastante incoveniente</font> <font COLOR="#231f20" size="2" face="Verdana">principalmente quando a população que será avaliada</font> <font COLOR="#231f20" size="2" face="Verdana">apresenta um padrão de atividade física mais intensa. Pessoas</font> <font COLOR="#231f20" size="2" face="Verdana">com elevada quantidade de massa muscular podem apresentar</font> <font COLOR="#231f20" size="2" face="Verdana">elevado IMC mesmo que a gordura corporal não seja excessiva</font> <font COLOR="#231f20" size="2" face="Verdana">(19).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Além disso, os pontos de corte propostos para o IMC,</font> <font COLOR="#231f20" size="2" face="Verdana">para avaliação do estado nutricional, são aplicados em uma</font> <font COLOR="#231f20" size="2" face="Verdana">faixa etária muito ampla, desconsiderando-se sexo e etnia.</font> <font COLOR="#231f20" size="2" face="Verdana">Sabe-se que, ao longo das décadas, ocorrem alterações</font> <font COLOR="#231f20" size="2" face="Verdana">fisiológicas na composição corporal, com aumento da</font> <font COLOR="#231f20" size="2" face="Verdana">quantidade de tecido adiposo e/ou redução da massa magra(20) e redução da massa óssea (21), especialmente entre as</font> <font COLOR="#231f20" size="2" face="Verdana">mulheres que têm a composição corporal diretamente afetada</font> <font COLOR="#231f20" size="2" face="Verdana">pelas alterações hormonais observadas na menopausa (22).</font> <font COLOR="#231f20" size="2" face="Verdana">Adicionalmente, como demonstrado por Deurenberg-Yap</font> <font COLOR="#231f20" size="2" face="Verdana">et al. (23), a relação entre IMC e gordura corporal difere</font> <font COLOR="#231f20" size="2" face="Verdana">significantemente entre grupos étnicos. Outros estudos</font> <font COLOR="#231f20" size="2" face="Verdana">demonstram a presença de alto risco cardiovascular em</font> <font COLOR="#231f20" size="2" face="Verdana">indivíduos com IMC normal (24). Na população asiática, os</font> <font COLOR="#231f20" size="2" face="Verdana">pesquisadores sugerem pontos de corte de IMC mais baixos,</font> <font COLOR="#231f20" size="2" face="Verdana">em torno de 27 kg/m², para diagnóstico de obesidade (25).</font></p>     ]]></body>
<body><![CDATA[<p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Muitos autores, ao fazerem críticas ao uso do IMC,</font> <font COLOR="#231f20" size="2" face="Verdana">propõem o uso do índice de massa de gordura e do índice de</font> <font COLOR="#231f20" size="2" face="Verdana">massa livre de gordura, e pontos de corte baseados na</font> <font COLOR="#231f20" size="2" face="Verdana">distribuição destes indicadores em percentis, porém, para a</font> <font COLOR="#231f20" size="2" face="Verdana">obtenção destes índices, é necessária avaliação da composição</font> <font COLOR="#231f20" size="2" face="Verdana">corporal, o que dificulta relativamente a sua utilização (26,</font> <font COLOR="#231f20" size="2" face="Verdana">27).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Outros parâmetros antropométricos, como circunferência</font> <font COLOR="#231f20" size="2" face="Verdana">da cintura, relação cintura-quadril e medida de dobras</font> <font COLOR="#231f20" size="2" face="Verdana">cutâneas, têm-se mostrado mais sensíveis que o IMC na</font> <font COLOR="#231f20" size="2" face="Verdana">identificação de indivíduos com excesso de gordura corporal</font> <font COLOR="#231f20" size="2" face="Verdana">e alterações metabólicas (28).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">A circunferência da cintura permite avaliar a distribuição</font> <font COLOR="#231f20" size="2" face="Verdana">central da gordura corporal, sendo uma medida qualitativa e</font> <font COLOR="#231f20" size="2" face="Verdana">não quantitativa. Atualmente, esta medida tem recebido</font> <font COLOR="#231f20" size="2" face="Verdana">importante atenção na avaliação do risco cardiovascular pelo</font> <font COLOR="#231f20" size="2" face="Verdana">fato de ser forte preditora da quantidade de gordura visceral,</font> <font COLOR="#231f20" size="2" face="Verdana">a principal responsável pelo aparecimento de alterações</font> <font COLOR="#231f20" size="2" face="Verdana">metabólicas e de doenças cardiovasculares (11, 29).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Assim como o IMC, a medida de circunferência da cintura</font> <font COLOR="#231f20" size="2" face="Verdana">também tem aumentado, nos últimos anos, na população</font> <font COLOR="#231f20" size="2" face="Verdana">americana. Segundo Estudo de Ford et al. (30), realizado a</font> <font COLOR="#231f20" size="2" face="Verdana">partir de dados do NHANES, a mediana da circunferência da</font> <font COLOR="#231f20" size="2" face="Verdana">cintura passou de 94,4 cm para 97,1 cm entre 1988 e 2000,</font> <font COLOR="#231f20" size="2" face="Verdana">confirmando rápido aumento da obesidade abdominal.</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Segundo Han et al. (31), os pontos de corte validados por</font> <font COLOR="#231f20" size="2" face="Verdana">Lean et al. (29), e atualmente preconizados pela WHO (5),</font> <font COLOR="#231f20" size="2" face="Verdana">foram capazes de identificar indivíduos com alto risco de</font> <font COLOR="#231f20" size="2" face="Verdana">doenças crônicas e apresentaram alta sensibilidade e</font> <font COLOR="#231f20" size="2" face="Verdana">especificidade na identificação de indivíduos classificados com</font> <font COLOR="#231f20" size="2" face="Verdana">sobrepeso e obesos pelo IMC. Outros estudos demonstram a</font> <font COLOR="#231f20" size="2" face="Verdana">validade desses pontos de corte em predizer alterações</font> <font COLOR="#231f20" size="2" face="Verdana">metabólicas e doenças como diabetes mellitus e hipertensão</font> <font COLOR="#231f20" size="2" face="Verdana">arterial (9, 15).</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">A relação cintura-quadril é um outro indicador associado</font> <font COLOR="#231f20" size="2" face="Verdana">a aumento do risco de doenças crônicas não-transmissíveis e</font> <font COLOR="#231f20" size="2" face="Verdana">que também têm aumentado significativamente nos últimos</font> <font COLOR="#231f20" size="2" face="Verdana">anos (8). No estudo de Lahti-Koski et al. (32), referente a um</font> <font COLOR="#231f20" size="2" face="Verdana">período de 10 anos, a RCQ passou de 0,907 para 0,925 em</font> <font COLOR="#231f20" size="2" face="Verdana">homens adultos.</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Os estudos demonstram que a RCQ é capaz de predizer a</font> <font COLOR="#231f20" size="2" face="Verdana">incidência de diabetes na mesma magnitude que outros índices,</font> <font COLOR="#231f20" size="2" face="Verdana">como IMC e circunferência da cintura (9), e associa-se</font> <font COLOR="#231f20" size="2" face="Verdana">inversamente melhor com baixos níveis de HDL do que a</font> <font COLOR="#231f20" size="2" face="Verdana">circunferência da cintura (10).</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">A RCQ também tem apresentado maior capacidade</font> <font COLOR="#231f20" size="2" face="Verdana">preditiva para hipertensão arterial que a circunferência da</font> <font COLOR="#231f20" size="2" face="Verdana">cintura em homens e mulheres acima de 20 anos (11). No</font> <font COLOR="#231f20" size="2" face="Verdana">estudo de Gus et al. (14), esta associação foi verificada somente</font> <font COLOR="#231f20" size="2" face="Verdana">em mulheres.</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">As variáveis antropométricas também têm sido utilizadas</font> <font COLOR="#231f20" size="2" face="Verdana">em equações de predição da gordura corporal oferecendomaior facilidade e baixo custo para as estimativas do percentual</font> <font COLOR="#231f20" size="2" face="Verdana">de gordura corporal em adultos. Deurenberg et al. (33)</font> <font COLOR="#231f20" size="2" face="Verdana">desenvolveram equações baseadas no IMC, na idade e no sexo,</font> <font COLOR="#231f20" size="2" face="Verdana">utilizando a pesagem hidrostática como método de referência.</font> <font COLOR="#231f20" size="2" face="Verdana">Gallagher et al. (34) também desenvolveram fórmulas para</font> <font COLOR="#231f20" size="2" face="Verdana">estimar a gordura corporal pelo IMC e outras variáveis como</font> <font COLOR="#231f20" size="2" face="Verdana">idade, sexo e etnia, utilizando o DEXA e o modelo de quatro</font> <font COLOR="#231f20" size="2" face="Verdana">compartimentos como métodos de referência.</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Além do IMC, a circunferência da cintura também tem</font> <font COLOR="#231f20" size="2" face="Verdana">sido utilizada para predizer a gordura corporal em adultos.</font> <font COLOR="#231f20" size="2" face="Verdana">Lean et al. (35) validaram equações de predição da gordura</font> <font COLOR="#231f20" size="2" face="Verdana">corporal tanto para homens quanto para mulheres, baseadas</font> <font COLOR="#231f20" size="2" face="Verdana">apenas na circunferência da cintura e na idade, utilizando a</font> <font COLOR="#231f20" size="2" face="Verdana">pesagem hidrostática como método de referência. A inclusão</font> <font COLOR="#231f20" size="2" face="Verdana">de medidas de dobras cutâneas foi avaliada pelos autores,</font> <font COLOR="#231f20" size="2" face="Verdana">entretanto, observou-se que as estimativas apresentaram</font> <font COLOR="#231f20" size="2" face="Verdana">maiores erros comparadas àquelas obtidas apenas por meio</font> <font COLOR="#231f20" size="2" face="Verdana">da circunferência da cintura.</font></p>     ]]></body>
<body><![CDATA[<p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">A vantagem de utilizar equações baseadas no IMC ou na</font> <font COLOR="#231f20" size="2" face="Verdana">circunferência da cintura reside no fato de que a estimativa do</font> <font COLOR="#231f20" size="2" face="Verdana">percentual de gordura é menos dependente de erros intra e</font> <font COLOR="#231f20" size="2" face="Verdana">inter-avaliador, comparada às das dobras cutâneas, método</font> <font COLOR="#231f20" size="2" face="Verdana">amplamente utilizado na avaliação da composição corporal.</font></p> <b>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Medidas de dobras cutâneas</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"></b><font COLOR="#231f20" size="2" face="Verdana">As medidas de dobras cutâneas tem sido o método mais</font> <font COLOR="#231f20" size="2" face="Verdana">utilizado na determinação da gordura corporal em razão do</font> <font COLOR="#231f20" size="2" face="Verdana">baixo custo operacional e da relativa simplicidade de</font> <font COLOR="#231f20" size="2" face="Verdana">utilização, em relação aos outros métodos de avaliação. Na</font> <font COLOR="#231f20" size="2" face="Verdana">literatura, são documentadas mais de 100 equações para</font> <font COLOR="#231f20" size="2" face="Verdana">estimar a gordura corporal que utilizam as medidas de dobras</font> <font COLOR="#231f20" size="2" face="Verdana">cutâneas e, normalmente, são divididas em específicas,</font> <font COLOR="#231f20" size="2" face="Verdana">validadas em grupos mais homogêneos e generalizadas com</font> <font COLOR="#231f20" size="2" face="Verdana">base em amostras com características heterogêneas (36).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Normalmente, a pesagem hidrostática é o método mais</font> <font COLOR="#231f20" size="2" face="Verdana">utilizado como referência para validação das equações</font> <font COLOR="#231f20" size="2" face="Verdana">envolvendo medidas de dobras cutâneas (37-41). Entretanto,</font> <font COLOR="#231f20" size="2" face="Verdana">nos últimos anos a sua utilização como método &quot;padrão-ouro&quot;</font> <font COLOR="#231f20" size="2" face="Verdana">vem sendo questionada, e outros métodos como o DEXA e o</font> <font COLOR="#231f20" size="2" face="Verdana">modelo de quatro compartimentos têm sido utilizados em</font> <font COLOR="#231f20" size="2" face="Verdana">estudos de validação (42,43). É importante ressaltar que as</font> <font COLOR="#231f20" size="2" face="Verdana">estimativas de gordura corporal obtidas por equações da</font> <font COLOR="#231f20" size="2" face="Verdana">literatura são válidas somente na população em que estas foram</font> <font COLOR="#231f20" size="2" face="Verdana">validadas. Para que sejam aplicadas em outras populações, é</font> <font COLOR="#231f20" size="2" face="Verdana">necessária prévia validação para verificar a acurácia das</font> <font COLOR="#231f20" size="2" face="Verdana">equações.</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Estudos verificando a validade de equações, como as de</font> <font COLOR="#231f20" size="2" face="Verdana">Durnin &amp; Wormersley (37) e de Jackson &amp; Pollock (39), em</font> <font COLOR="#231f20" size="2" face="Verdana">outros grupos populacionais, têm demonstrado baixa acurácia</font> <font COLOR="#231f20" size="2" face="Verdana">devido a diferenças inerentes à etnia (44), ao nível de atividade</font> <font COLOR="#231f20" size="2" face="Verdana">físico (45) e à quantidade de gordura corporal (42).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">No Brasil, dois estudos validaram equações baseadas em</font> <font COLOR="#231f20" size="2" face="Verdana">medidas de dobras cutâneas utilizando a pesagem hidrostática</font> <font COLOR="#231f20" size="2" face="Verdana">como método de referência; um envolveu 206 estudantes</font> <font COLOR="#231f20" size="2" face="Verdana">universitários do Sul do País (40) e o outro avaliou adultos,</font> <font COLOR="#231f20" size="2" face="Verdana">com idade entre 18 e 60 anos, desta mesma região brasileira(41).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Costa (26) estudou uma amostra representativa da cidade</font> <font COLOR="#231f20" size="2" face="Verdana">de Santos (SP) e descreveu o comportamento da quantidade e</font> <font COLOR="#231f20" size="2" face="Verdana">distribuição da gordura corporal de acordo com o sexo e a</font> <font COLOR="#231f20" size="2" face="Verdana">idade por meio de curvas de percentis, definindo como faixa</font> <font COLOR="#231f20" size="2" face="Verdana">normal de gordura corporal os valores dispostos entre os</font> <font COLOR="#231f20" size="2" face="Verdana">percentis 25 e 75, e como categoria de maior risco</font> <font COLOR="#231f20" size="2" face="Verdana">cardiovascular o percentil acima de 90.</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Outros estudos envolvendo a população brasileira têm</font> <font COLOR="#231f20" size="2" face="Verdana">realizado a validação de diversas equações, entretanto, observase</font> <font COLOR="#231f20" size="2" face="Verdana">que, na maioria das vezes, as amostras são pequenas ou</font> <font COLOR="#231f20" size="2" face="Verdana">envolvem grupos muito específicos da população, como</font> <font COLOR="#231f20" size="2" face="Verdana">militares, universitários e mulheres pós-menopausa (46,47).</font> <font COLOR="#231f20" size="2" face="Verdana">Apesar de existirem equações validadas no Brasil, observa-se</font> <font COLOR="#231f20" size="2" face="Verdana">ainda o predomínio da utilização das equações de Jackson &amp;</font> <font COLOR="#231f20" size="2" face="Verdana">Pollock (38) e Durnin &amp; Wormersley (37). Segundo o estudo</font> <font COLOR="#231f20" size="2" face="Verdana">de Petroski (41), para os homens brasileiros, apenas três das</font> <font COLOR="#231f20" size="2" face="Verdana">oito equações elaboradas por Jackson &amp; Pollock (38) foram</font> <font COLOR="#231f20" size="2" face="Verdana">válidas. As equações específicas e generalizadas de Durnin &amp;</font> <font COLOR="#231f20" size="2" face="Verdana">Wormersley (37) subestimaram significantemente o percentual</font> <font COLOR="#231f20" size="2" face="Verdana">de gordura corporal mensurado por pesagem hidrostática.</font></p> <b>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Bioimpedância elétrica</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"></b><font COLOR="#231f20" size="2" face="Verdana">A análise da composição corporal por meio da</font> <font COLOR="#231f20" size="2" face="Verdana">bioimpedância elétrica (BIA) é baseada na condução de uma</font> <font COLOR="#231f20" size="2" face="Verdana">corrente elétrica indolor, de baixa intensidade, aplicada ao</font> <font COLOR="#231f20" size="2" face="Verdana">organismo por meio de cabos conectados a eletrodos ou</font> <font COLOR="#231f20" size="2" face="Verdana">superfícies condutoras, que são colocados em contato com a</font> <font COLOR="#231f20" size="2" face="Verdana">pele. A impedância, dada pelos valores de reactância e</font> <font COLOR="#231f20" size="2" face="Verdana">resistência, é baixa no tecido magro, onde se encontram,</font> <font COLOR="#231f20" size="2" face="Verdana">principalmente, os líquidos intracelulares e eletrólitos, e alta</font> <font COLOR="#231f20" size="2" face="Verdana">no tecido adiposo (48).</font></p>     ]]></body>
<body><![CDATA[<p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">A BIA, apesar de sua facilidade técnica e alta</font> <font COLOR="#231f20" size="2" face="Verdana">reprodutibilidade, pode resultar em estimativas menos precisas</font> <font COLOR="#231f20" size="2" face="Verdana">nas situações em que o balanço hidroeletrolítico está alterado.</font> <font COLOR="#231f20" size="2" face="Verdana">Portanto, fatores como ingestão de álcool e atividade física</font> <font COLOR="#231f20" size="2" face="Verdana">intensa realizada antes do teste e presença de edema ou retenção</font> <font COLOR="#231f20" size="2" face="Verdana">hídrica em certos períodos do ciclo menstrual devem ser</font> <font COLOR="#231f20" size="2" face="Verdana">avaliado (49). Além disso, outros fatores como ingestão recente</font> <font COLOR="#231f20" size="2" face="Verdana">de alimentos (50) e obesidade (51) também podem prejudicar</font> <font COLOR="#231f20" size="2" face="Verdana">a acurácia do teste.</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Os aparelhos disponíveis para este tipo de avaliaçãofornecem os valores de massa de gordura, massa magra e água</font> <font COLOR="#231f20" size="2" face="Verdana">corporal por meio de equações preditivas ajustadas para sexo,</font> <font COLOR="#231f20" size="2" face="Verdana">idade, peso, altura e nível de atividade física. Entretanto, as</font> <font COLOR="#231f20" size="2" face="Verdana">equações de predição variam conforme o aparelho e da mesma</font> <font COLOR="#231f20" size="2" face="Verdana">forma que as equações de medidas de dobras cutâneas,</font> <font COLOR="#231f20" size="2" face="Verdana">apresentam validade apenas para a população de origem,</font> <font COLOR="#231f20" size="2" face="Verdana">necessitando, portanto, de validação cruzada em outros grupos</font> <font COLOR="#231f20" size="2" face="Verdana">populacionais.</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Estudos de desenvolvimento e de validação de equações</font> <font COLOR="#231f20" size="2" face="Verdana">de BIA vêm sendo desenvolvidos (52, 53) e diversos aparelhos</font> <font COLOR="#231f20" size="2" face="Verdana">de BIA bipolar (mensuração em membros inferiores ou</font> <font COLOR="#231f20" size="2" face="Verdana">superiores) e tetrapolar (corpo inteiro) estão disponíveis no</font> <font COLOR="#231f20" size="2" face="Verdana">mercado. Entretanto, apesar da facilidade técnica, da alta</font> <font COLOR="#231f20" size="2" face="Verdana">reprodutibilidade, do custo relativamente baixo e por ser um</font> <font COLOR="#231f20" size="2" face="Verdana">método não invasivo, estudos têm apresentado valores de</font> <font COLOR="#231f20" size="2" face="Verdana">composição corporal significantemente diferentes dos obtidos</font> <font COLOR="#231f20" size="2" face="Verdana">por pesagem hidrostática ou DEXA (54), enquanto outros não</font> <font COLOR="#231f20" size="2" face="Verdana">o fazem (55,56).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">As controvérsias entre os estudos podem ser devidas à</font> <font COLOR="#231f20" size="2" face="Verdana">variabilidade das equações e dos equipamentos utilizados, de</font> <font COLOR="#231f20" size="2" face="Verdana">adoção de diferentes protocolos e freqüências, de diferenças</font> <font COLOR="#231f20" size="2" face="Verdana">étnicas e de composição corporal entre as populações, além</font> <font COLOR="#231f20" size="2" face="Verdana">de influências no estado de hidratação.</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Estudos envolvendo a comparação de métodos</font> <font COLOR="#231f20" size="2" face="Verdana">demonstram que, comparada às equações antropométricas,</font> <font COLOR="#231f20" size="2" face="Verdana">como as de Durnin &amp; Wormersley (37) e as de Jackson &amp;</font> <font COLOR="#231f20" size="2" face="Verdana">Pollock (38), a BIA tetrapolar produz estimativas com menores</font> <font COLOR="#231f20" size="2" face="Verdana">erros (56).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">No Brasil, Marques et al. (57) verificaram que a utilização</font> <font COLOR="#231f20" size="2" face="Verdana">das equações de Lohman (58) e de Stolarczyk et al. (56) é</font> <font COLOR="#231f20" size="2" face="Verdana">aceitável na avaliação da composição corporal de mulheres</font> <font COLOR="#231f20" size="2" face="Verdana">brasileiras jovens. No estudo de Glaner (59), somente a</font> <font COLOR="#231f20" size="2" face="Verdana">equação de Segal et al. (55), desenvolvida para indivíduos</font> <font COLOR="#231f20" size="2" face="Verdana">com percentual de gordura corporal maior que 20%,</font> <font COLOR="#231f20" size="2" face="Verdana">apresentou validade em homens militares brasileiros. Já no</font> <font COLOR="#231f20" size="2" face="Verdana">estudo de Carvalho &amp; Pires-Neto (60), também realizado no</font> <font COLOR="#231f20" size="2" face="Verdana">Brasil, constatou-se que as estimativas de gordura corporal</font> <font COLOR="#231f20" size="2" face="Verdana">foram superestimadas tanto em homens quanto em mulheres</font> <font COLOR="#231f20" size="2" face="Verdana">pela bioimpedância elétrica.</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Os três estudos brasileiros citados demonstram a</font> <font COLOR="#231f20" size="2" face="Verdana">dificuldade de se estabelecer alguma conclusão sobre a</font> <font COLOR="#231f20" size="2" face="Verdana">validade da BIA em brasileiros devido às diferenças entre os</font> <font COLOR="#231f20" size="2" face="Verdana">grupos avaliados e ao fato de diferentes equações terem sido</font> <font COLOR="#231f20" size="2" face="Verdana">utilizadas, além da escassez de estudos desta natureza no País.</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Com relação à BIA bipolar, a maioria dos estudos</font> <font COLOR="#231f20" size="2" face="Verdana">demonstra resultados favoráveis ao seu uso. No estudo de</font> <font COLOR="#231f20" size="2" face="Verdana">Swartz et al. (261), não houve nenhuma diferença significante</font> <font COLOR="#231f20" size="2" face="Verdana">entre o percentual de gordura corporal obtido pela BIA bipolar</font> <font COLOR="#231f20" size="2" face="Verdana">(Tanita TBF-305®) e o determinado pela pesagem hidrostática,</font> <font COLOR="#231f20" face="Verdana" size="2">em homens, quando a intensidade da atividade física foi levada</font> <font COLOR="#231f20" size="2" face="Verdana">em consideração. Entretanto, os autores demonstraram erros</font> <font COLOR="#231f20" size="2" face="Verdana">de estimativa individual importantes.</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Jebb et al. (62) e Cable et al. (63) também verificaram que</font> <font COLOR="#231f20" face="Verdana" size="2"> a BIA bipolar (Tanita TBF-305 e TBF-105®, respectivamente)</font> <font COLOR="#231f20" face="Verdana" size="2">foi válida na estimativa da composição corporal de homens</font> <font COLOR="#231f20" size="2" face="Verdana">adultos. Utter et al. (64), estudando mulheres eutróficas e</font> <font COLOR="#231f20" size="2" face="Verdana">obesas, não encontraram diferenças significantes entre o</font> <font COLOR="#231f20" size="2" face="Verdana">percentual de gordura obtido por BIA bipolar (Tanita TBF-</font> <font COLOR="#231f20" size="2" face="Verdana">105®) e pesagem hidrostática nos dois grupos.</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" face="Verdana" size="2">Já no estudo de Lukaski &amp; Siders (65), tanto a BIA bipolar</font> <font COLOR="#231f20" size="2" face="Verdana">superior (Omron HBF-301®) quanto a inferior (Tanita TBF-</font> <font COLOR="#231f20" face="Verdana" size="2">105®) não foram adequadas para a avaliação da gordura</font> <font COLOR="#231f20" face="Verdana" size="2">corporal de homens e mulheres com idade entre 21 e 60 anos,</font> <font COLOR="#231f20" size="2" face="Verdana">comparadas ao DEXA. Portanto, como ocorre com a BIA</font> <font COLOR="#231f20" size="2" face="Verdana">tetrapolar, os estudos com a BIA bipolar, apesar de a indicarem</font> <font COLOR="#231f20" size="2" face="Verdana">na avaliação da composição corporal, necessitam de validação</font> <font COLOR="#231f20" size="2" face="Verdana">cruzada na população brasileira.</font> </p>     ]]></body>
<body><![CDATA[<p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Com relação à validade da bioimpedância para detectar</font> <font COLOR="#231f20" size="2" face="Verdana">modificações na composição corporal em programas de</font> <font COLOR="#231f20" size="2" face="Verdana">intervenção, foi verificado no estudo de Jebb et al. (66) que a</font> <font COLOR="#231f20" size="2" face="Verdana">estimativa de mudanças na gordura corporal pela BIA bipolar</font> <font COLOR="#231f20" size="2" face="Verdana">relativa ao modelo de multicompartimentos foi similar às</font> <font COLOR="#231f20" size="2" face="Verdana">estimativas dadas por outros métodos como plestimografia,</font> <font COLOR="#231f20" size="2" face="Verdana">DEXA e água corporal total (diluição com deutério), e foi</font> <font COLOR="#231f20" size="2" face="Verdana">melhor que a bioimpedância tetrapolar.</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">No estudo de Alvarez et al. (67), a BIA tetrapolar não</font> <font COLOR="#231f20" size="2" face="Verdana">apresentou resultados satisfatórios na avaliação de indivíduos</font> <font COLOR="#231f20" size="2" face="Verdana">submetidos à perda de peso. Comparada ao DEXA a BIA</font> <font COLOR="#231f20" size="2" face="Verdana">apresentou erros de estimativas do percentual de gordura</font> <font COLOR="#231f20" size="2" face="Verdana">corporal inaceitáveis com erro padrão de +5,22% usando a</font> <font COLOR="#231f20" size="2" face="Verdana">equação de Lukaski e +7,81% quando a equação de Segal foi</font> <font COLOR="#231f20" size="2" face="Verdana">utilizada. A estimativa utilizando as medidas de dobras</font> <font COLOR="#231f20" size="2" face="Verdana">cutâneas foi muito mais precisa e acurada com um erro padrão</font> <font COLOR="#231f20" size="2" face="Verdana">de +0,86.</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Os métodos padrão utilizados, as características da amostra</font> <font COLOR="#231f20" size="2" face="Verdana">e o tempo de intervenção diferem entre os estudos, dificultando</font> <font COLOR="#231f20" size="2" face="Verdana">o estabelecimento de um consenso a cerca do uso da BIA na</font> <font COLOR="#231f20" size="2" face="Verdana">avaliação de modificações da composição corporal e</font> <font COLOR="#231f20" size="2" face="Verdana">demonstrando a necessidade de maiores investigações.</font></p> <b>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Interactância de infravermelho próximo 68</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"></b><font COLOR="#231f20" size="2" face="Verdana">A interactância de infravermelho próximo (NIR) é baseada</font> <font COLOR="#231f20" size="2" face="Verdana">em diferentes densidades ópticas obtidas por meio da absorção</font> <font COLOR="#231f20" size="2" face="Verdana">e reflexão de comprimentos de onda dos raios infravermelhos</font> <font COLOR="#231f20" size="2" face="Verdana">emitidos pelo aparelho no tecido subcutâneo de um</font> <font COLOR="#231f20" size="2" face="Verdana">determinado local anatômico, normalmente o bíceps (68).</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Inicialmente, este método foi utilizado com a finalidade</font> <font COLOR="#231f20" size="2" face="Verdana">de avaliar a composição química de alimentos. Posteriormente,</font> <font COLOR="#231f20" size="2" face="Verdana">foi modificado e adaptado para avaliação da composição</font> <font COLOR="#231f20" size="2" face="Verdana">corporal em seres humanos. Apesar de ser também um método</font> <font COLOR="#231f20" size="2" face="Verdana">de baixo custo, não invasivo, fácil e portátil, sua aplicação é</font> <font COLOR="#231f20" size="2" face="Verdana">questionável pelo fato de ter sido validado em uma amostra</font> <font COLOR="#231f20" size="2" face="Verdana">muito pequena (n=17) e homogênea e por predizer a gordura</font> <font COLOR="#231f20" size="2" face="Verdana">corporal a partir de um único local anatômico de depósito</font> <font COLOR="#231f20" size="2" face="Verdana">subcutâneo. Além disso, seus autores utilizaram apenas testes</font> <font COLOR="#231f20" size="2" face="Verdana">de correlação para verificar a concordância com o método de</font> <font COLOR="#231f20" size="2" face="Verdana"> diluição de deutério, utilizado como método de referência no</font> <font COLOR="#231f20" size="2" face="Verdana">estudo (68).</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Considerando métodos de avaliação da composição</font> <font COLOR="#231f20" size="2" face="Verdana">corporal, há na literatura relativamente poucos estudos desta</font> <font COLOR="#231f20" size="2" face="Verdana">natureza com o método NIR. A reprodutibilidade das medidas</font> <font COLOR="#231f20" size="2" face="Verdana">fornecidas pelo Futrex-5000® foi avaliada no estudo de</font> <font COLOR="#231f20" face="Verdana" size="2">Schreiner et al. (69) em que se verificaram diferenças</font> <font COLOR="#231f20" size="2" face="Verdana">estatisticamente significantes entre as medidas inter e intraavaliador.</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Estudos têm demonstrado que o NIR tende a superestimar</font> <font COLOR="#231f20" size="2" face="Verdana">a gordura corporal em indivíduos com percentuais mais baixos</font> <font COLOR="#231f20" size="2" face="Verdana">de gordura corporal e a subestimá-la em indivíduos com</font> <font COLOR="#231f20" size="2" face="Verdana">percentuais mais elevados (70,71). Em meta-análise de onze</font> <font COLOR="#231f20" size="2" face="Verdana">estudos, realizada por Fogelholm &amp; Lichtenbelt (72)</font> <font COLOR="#231f20" size="2" face="Verdana">constataram que o método NIR subestimou a gordura corporal</font> <font COLOR="#231f20" size="2" face="Verdana">em indivíduos com percentual de gordura corporal maior que25%, e muito mais em indivíduos obesos.</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">No estudo de Mclean &amp; Skinner (73) apenas 52% dos</font> <font COLOR="#231f20" size="2" face="Verdana">indivíduos avaliados se mantiverem ± 4% da média de</font> <font COLOR="#231f20" size="2" face="Verdana">percentual de gordura corporal obtida pelo método de pesagem</font> <font COLOR="#231f20" size="2" face="Verdana">hidrostática. Já no estudo de Thomas et al. (74), a subestimação</font> <font COLOR="#231f20" size="2" face="Verdana">média do NIR foi de 15%.</font> </p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">Hicks et al. (75) avaliaram a validade do Futrex-5000® em</font> <font COLOR="#231f20" face="Verdana" size="2">predizer a gordura corporal de mulheres com idade média de</font> <font COLOR="#231f20" size="2" face="Verdana">34,4 anos e verificaram que o método NIR subestimou</font> <font COLOR="#231f20" size="2" face="Verdana">significantemente o percentual em 4,4%. Uma das explicações</font> <font COLOR="#231f20" size="2" face="Verdana">para tal resultado foi o fato de 75% das mulheres terem sido</font> <font COLOR="#231f20" size="2" face="Verdana">obesas. Além disso, os autores relataram que diferenças na</font> <font COLOR="#231f20" size="2" face="Verdana">cor da pele podem afetar a mensuração da densidade óptica;</font> <font COLOR="#231f20" size="2" face="Verdana">indivíduos com pele mais escura tendem a apresentar menores</font> <font COLOR="#231f20" size="2" face="Verdana">valores de densidade óptica.</font></p> <b>     ]]></body>
<body><![CDATA[<p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font COLOR="#231f20" size="2" face="Verdana">CONSIDERAÇÕES FINAIS</font></p>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"></b><font COLOR="#231f20" size="2" face="Verdana">Apesar dos numerosos métodos de avaliação da composição</font> <font COLOR="#231f20" size="2" face="Verdana">corporal, são poucos os estudos que avaliam a sua</font> <font COLOR="#231f20" size="2" face="Verdana">reprodutibilidade e acurácia na população. Além disso, uma</font> <font COLOR="#231f20" size="2" face="Verdana">questão bastante discutida refere-se ao estabelecimento de pontos</font> <font COLOR="#231f20" size="2" face="Verdana">de corte de gordura corporal adequados para a identificação de</font> <font COLOR="#231f20" size="2" face="Verdana">indivíduos sob risco de alterações metabólicas.
